겨드랑이포트까지 제거하고 보형물교체한 가슴성형재수술<--밑선찝기도

이분은 좀 특이한 보형물을 넣으신 분입니다.

겨드랑이에 있는 포트를 통해서 식염수를 넣다 뺐다하면서 보형물사이즈를 조절할 수 있는 거죠.
10년전에 수술하셨고요.

위 초음파상에서도 보듯이, 유두유륜복합체와 유방밑선까지의 거리가 꽤 긴 분이었습니다.

즉, 보형물이 상당히 아래쪽에 위치해 있는 거죠.

가슴을 풍만하게 보이게 하는 효과는 있지만, 너무 아래에 있으면 또 가슴모양이 좀 어색해 집니다.

하방변위죠. 밑이 빠졌다는 표현도 쓰고요.

제가 그림으로 그려보았는데, 위 그림처럼 밑선까지의 거리가 긴 겁니다.

이런 경우는 중간에 새로운 유방밑선을 잡아줘야 합니다.

원래 이런 새로운 밑선을 잡으려면 밑선 절개를 해야 하는데, 그러면 또 흉터가 잘 보일 수 있어서,

저희는 유륜절개로 해서 속에서 저 밑선을 찝어 줍니다.

한두군데 찝으면 다시 확 내려가 버려서, 속에 피막과 피막을 겹쳐서 밑선을 찝어주는 방법을 씁니다.

역시 부분마취로 하지만, 이거할 때 시간이 좀 걸립니다. ㅠㅠ 30분 좀 넘게 걸리는 것 같습니다.

위 사진은 유방엑스레이인데 겨드랑이쪽에 포트(port)가 보입니다.

왼쪽은 포트가 안 만져 졌는데, 포트가 겨드랑이 속으로 또 너무 들어갔더군요. 그러면 또 위험해 집니다. 그 밑에는 큰 혈관이나 림프절 등의 위험한 구조물들이 많거든요.

게다가 이분 왼쪽 겨드랑이 포트는 뒤집어져서 겉에서 주사기로 식염수를 더 넣을 수도 없는 상황이었습니다.

한쪽 가슴보형물을 파손시켜서 속의 생리식염수를 뺐는데 위 사진처럼 노란색으로 변색되어 있었습니다.

겨드랑이 포트까지 연결한 온전한 모습의 생리식염수백입니다.

뒤집어 보니 바닥에 멘토 회사 마크가 보입니다.

275cc라는 표식도 보입니다.

이분 유방밑선을 올리면서 가슴보형물교체를 해서, 275cc에서 175cc로 100cc나 줄여서 넣어는데도 이상하지 않았습니다.

그냥 저 방을 썼다면 너무 방이 커서 속에서 돌아다녔겠죠.

밑선 찝어서 올리느라고, 하방변위 교정하느라고 고생은 했지만, 환자분이 부분마취로 하는 동안에 긴 시간동안 잘 참아주셔서, 저도 편안히 성공적으로 했습니다. ^^

가슴성형재수술이 쉽지 않지만, 되도록 사람 몸을 많이 조작하지 않고, 조금만 건드리고 기존의 조직을 최대한 활용하는 방식으로 하니까 부분마취로 가능하더군요. ^^::
