캐나다에서 오신 파손된 옛날실리콘가슴보형물제거수술후기

이분은 캐나다에서 오셨는데,

25년전 명동의 어떤 성형외과에서 당시 실리콘 가슴보형물(옛날 실리콘, 3세대)로 겨드랑이절개로 확대성형수술하신 분입니다.

당시에는 20대 이셨지만 지금은 50대가 되셔서 날짜가 좀 지났으니 이제는 가슴보형물을 제거하고 싶어서 오셨습니다.

그런데...

수술전 검사로서 유방엑스레이(유방촬영술, mammography)와 유방초음파를 보니 위 사진처럼 양쪽 가슴보형물이 다 터져 있더군요. 파손파열된 것이 명확하였습니다. 그날 바로 제거하였습니다.

부분마취니까 금식하실 필요도 없고, 미리 가슴보형물제거수술을 하기로 예약하셨기 때문에 수술시간도 비워져 있었습니다. 다만 이렇게 파손된 경우는 닦아내느라고 시간이 좀 더 걸리기는 합니다. 환자분이 더 아프신 건 아니고요.

요새는 이렇게 미리 수술까지 예약하시고 당일 바로 수술하시는 분들이 많습니다. 워낙 저희 병원 예약잡기가 쉽지 않으니... 멀리서 오신 김에 한번에 해결하려고들 생각하시더군요. 오히려 서울에 사시는 분들이 오시면 반가울 지경이라고 해야 하나... 정말 지방에서 많이 오십니다. 이분처럼 외국에서 오시는 분들도 많고요.

제주도나 부산에서는 비행기 타고 많이 오시고, 지하철 9호선 급행타고 오시면 신논현역까지 금방 오고, 광주나 전주 쪽에서는 SRT타고 수서역에서 내리셔서 지하철타고 오시더군요. 수서역에서 분당선타고 선정릉역에서 9호선으로 갈아타서 신논현역으로 오시는 게 제일 빠르다고 합니다. 환자분들 말씀이. 대구에서 오시는 분들도 SRT를 타고 오셨는데... KTX가 아니라.
강남역보다는 신논현역에서 좀 더 가깝고, 강남역이 워낙 사람이 많아서 정신 없다 보니 신논현역이 좀 더 시간이 단축되나 봅니다.

아무튼... 이분도 부분마취하고 가슴보형물을 터지지 않게, 더 터져 버리지 않게 조심스레 제거하였습니다. 터졌지만 구형구축은 없었습니다. 가슴성형재수술하기에 나쁘지 않은 환경이었죠.

위 사진처럼 파손되어서 번져나온 실리콘으로 인해 뒤의 종이가 좀 젖어 있는 게 보입니다.

피가 안 나게 하는 게 저희 병원의 노하우이고요. ^^

뒤집어 보니 Silastic II 라고 쓰여 있고 135cc였습니다.

이런 것은 bleed라고 부르는데 피가 흐른다는 의미가 아니라, 옛날 실리콘같은 경우는 오래 되면 직접적인 파손이 없다 할 지라도, 보형물껍질을 새어나오는 실리콘 내용물들이 생기게 됩니다. 이런 걸 bleed라고 표현하는 현상입니다.

즉, 또렷이 파손된 부분은 찾지 못 하지만 표면에 끈적거리는 실리콘내용물들이 뭍어 있는 거죠.

25년 되었으니 이렇게 파손된 게 사실 당연하다고도 볼 수 있을 겁니다. 이것도 공산품이니 언젠가는 제거나 교체를 해야 하겠죠.

1주일후 오셔서 체크초음파를 해 보니 위 사진처럼 많은 양의 장액과 혈종이 있어서,

저 바로 밑에는 심장과 폐가 있어서 위험합니다. 절대 그냥 못 하고 초음파로 보면서 해야 합니다. 그래도 위험해서 정말 조심해서 해야 합니다.

어떻게 보면 가슴보형물제거수술은 이런 후관리가 정말 중요합니다.

그냥두면 피막이 점점 두꺼워지고, 고여있는 장액종과 혈종은 감염의 우려가 있기 때문입니다.

이건 성형외과에서는 못 해 주십니다. 그래서 다른 병원에서 가슴보형물제거하신 후 장액종이 지속적으로 고여서 저희 병원 오셔서 초음파 보면서 뽑으시는 분들 많습니다.
이렇게 사후처리에 특화된 유방외과이다보니 이런 당연한 것이 다른 병원에서는 너무나 어렵고 신기한 일이 되기도 하고요.

위 사진처럼 많은 양을 뽑았습니다. 한쪽당 30cc 는 나온 것 같습니다.
